Online Fundraiser Store

Please fill in the information below to help us set up your online fundraiser store.

Contact name – the person we will send all correspondence including e-mail, mail, and telephone if we need to contact you directly.
 
 
E-mail address – e-mail address of the above contact name. Most correspondence is sent through e-mail including an e-mail to let you know when your teams online store is active and an attached custom sales flyer that you can print, copy, and distribute to all team members, family, and friends to promote your fundraiser.
 
 
Shipping choice – each order shipped directly to the customer or ship all the products ordered to the above contact at the end of the fundraiser.
 
 
Mailing/shipping address – address where the products will be shipped when “One shipment after fundraiser” is chosen as a shipping option. Also, the address the fundraiser check will be sent for your portion of all sales.

Pay to the order of – enter the name that should appear on the check for your 20% portion of the fundraising sales.
 
 
Dates – enter the dates that you would like your online fundraising store to be open. Please allow 5 business days for us to prepare your online store. We will e-mail you when your store is open for business.

Sport – enter the team sport you coach.
 
 
Team name – name that will appear on all your custom team products.
 
 
Team colors – choose the main color that will be used on the team name text and the secondary outline color that will be used on the text.

Team Logo (optional) – Click the browse button.  Select "all files *.*" from the file type drop-down menu and choose the image file for your team's logo.

Reason for Fundraiser – enter a description of what the fundraiser is for so we can include it on your custom sales flyer.
 
 
Additional info – enter any additional information that you would like us to know.
